- E-mail or give the printed Volunteer Reference form to two people NOT related to you and have them complete the reference within one week of the date you gave it to them.
- SIGNATURE MUST BE INCLUDED
Fill out both the Volunteer Application and the Volunteer Reference form COMPLETELY and send them to:
